A case of peduncular hallucinosis due to a pontine infarction: a rare complication of coronary angiography.

چکیده
BACKGROUND Cerebral thromboembolism is a rare, but well-recognized complication of angiographic procedures. Peduncular hallucinosis (PH) is a form of complex visual hallucinations usually associated with lesions in the midbrain and thalamus. CASE PRESENTATION We report the case of a 79-years-old male patient with internuclear ophthalmoplegia and vivid lilliputian visual hallucinations (peduncular hallucinations), caused by a pontine infarction following coronary artery catheterization. The patient was started on quetiapine treatment with good results and tolerance. In the next three months, the medication has been discontinued, and the patient is without symptomatology thereafter. CONCLUSION An understanding of how different pathologies may produce complex visual hallucinations can lead to an appropriate treatment, depending on the site and the nature of the lesion. Furthermore, cerebral embolism due to any angiographic procedure, although rare, should always be taken into consideration, upon any neurological manifestation, visual hallucinations included. The authors present a unique case of intracranial lipoma in the interpeduncular cistern associated with proximal P1 segment fenestration. This patient is a 20-year-old male with extensive psychiatric history and complaints of recent auditory hallucinations. Cranial magnetic resonance imaging (MRI) (T1, T2, and FLAIR) showed a hyperintense lesion in the left aspect of interpeduncular cistern wit...
متن کاملCongenital Absence of Left Circumflex Coronary Artery
Congenital absence of left circumflex artery is a rare congenital anomaly of the coronary arteries. The prevalence of the anomaly in different studies ranges from 0.6% to 1.3%. Of these, 80% are benign and asymptomatic and 20% are clinically important. We report a 56-year-old man presented with acute resting chest pain who was diagnosed as having acute anterolateral infarction accompanied by el...

Central Pontine Myelinosis (CPM) and Extra Pontine Myelinosis (EPM) are debilitating conditions, resulting from the rapid correction of hyponatremia. The outcome and prognosis of this medical condition are variable; mainly involving functional decline from neuro-musculoskeletal upset. An intensive, multidisciplinary rehabilitation has played an essential role in facilitating and hasten recovery...
متن کاملA potential case of peduncular hallucinosis treated successfully with olanzapine.
Visual hallucinations have a differential diagnosis, both psychiatric and nonpsychiatric in nature. Described first by Lhermitte, peduncular hallucinosis is an uncommon etiology of visual hallucinations (VH). Typically, the offending lesion is vascular in origin and occurs at the level of the midbrain, thalamus, or rostral brainstem. Interestingly, the origin of the VH in our patient's case cou...

Coronary aneurysms in adults are rare clinical entities. Herein, we presented a 54-year-old man who was admitted with an acute extensive anterior myocardial infarction. The emergency coronary angiography revealed an isolated large aneurysm at the proximal segment of the left anterior descending coronary artery. The patient was successfully treated with a graft-coated stent
